I have a 21' Bayliner Rendezvous boat with a 1998 Mercury 150 with a 20" shaft on the boat. The outboard has very low compression on one cylinder and the others are low as well. I have recently purchased a 2004 Yamaha 150 hpdi with a 25" shaft. Will a jack plate with the ability to raise the motor that 5" be sufficient and will that work with my boat?

Thank you in advance for any and all advice.

If your motor is mounted at it's very lowest possible position flat on the transom, you may get away with it. But as eli indicated it will not likely be ideal. Also your transom may not be designed for a jack plate lifted up to max height all the time. Puts a lot of strain on the transom.
